If the browser displays the application document, click on the diskette icon to save on disk.

Note:

Microsoft Word format require Microsoft Word software or any other word processing software capable of converting Microsoft Word format.

Adobe PDF format requires Adobe Acrobat Reader Software. This software is not a word processor. Use the Acrobat Reader to print the application, then fill out the application and fax it to the number provided. Adobe Acrobat Reader software can be download for free from Adobe.
